The department is established to provide assistance in social and domestic, social and labor, and social and psychological rehabilitation and habilitation of disabled persons and elderly citizens in the conditions of day care at the department and is responsible for providing consulting and information, social and domestic, social intermediary, social and psychological, social rehabilitation services, personal assistant services:

– disabled persons of Group I and II;

– disabled persons of Group III among unemployed citizens whose disability was established for the first time, for up to three consecutive months within a year from the date of disability establishment;

– unemployed citizens aged 60 and older who have reached the generally established retirement age and are eligible for state pension.

Areas of Department Activity (for work with disabled persons):

identification and differentiated accounting of relevant citizens;
comprehensive assessment of the need of disabled persons for social rehabilitation and habilitation;
provision of day care for disabled persons;
participation in the work of the medico-rehabilitation expert commission;

participation in the formation of the social rehabilitation and habilitation section of the individual rehabilitation and habilitation program for persons with disabilities (program);
development of individual social rehabilitation and habilitation plans for persons with disabilities based on the program, selection of the necessary list and volume of social services, other types of social rehabilitation and habilitation, participation in assessing their effectiveness;
carrying out work with citizens in accordance with medical prescriptions for occupational therapy;
development of abilities, interests and vocational skills in persons with disabilities, ensuring the realization of their potential employment opportunities;
involvement of persons with disabilities, taking into account their capabilities, in various forms of social life, creative activities, physical culture and health-improvement events;
formation of communication skills, practical skills and abilities in persons with disabilities, ensuring autonomy in everyday life;
assistance to persons with disabilities in restoring self-care skills and preparing for independent living;
organization and conduct of individual and group therapeutic physical training sessions as prescribed by a doctor;
continuous monitoring of persons with disabilities, control over their physical condition;
conducting classes with persons with disabilities, including at home, within the framework of interest clubs.

To conclude a contract for the provision of social services, a citizen must submit the following documents:

identity document of the citizen;
a document of the established sample on the right to benefits (disability certificate) for citizens belonging to the category of beneficiaries;
consent to the processing of special personal data in cases provided for by personal data legislation;
written application (established sample);
medical certificate of health status and (or) conclusion of the medical advisory commission containing information about the absence of medical contraindications for the provision of social services in the form of semi-inpatient social services;
accompanying documents from correctional and developmental education and rehabilitation centers – for graduates of the said centers.

Within 3 business days from the date of application, employees of the territorial center shall conduct an assessment of the citizen's living conditions, draw up an act determining the individual need for social services, and request the following information from state bodies and state organizations:

information about the residential premises occupied in the given locality, place of residence and family composition (including information about the place of residence and family composition) – regarding the person applying for social services;
information on the amount of pension received for the month preceding the month of application for social services – for single incapacitated citizens (excluding children with disabilities under 18 years of age) who have applied for social services.

To conclude a contract for the provision of social services "Conducting classes on the formation, and/or restoration, and/or development of social skills", "Organization of hobby clubs", group III disabled individuals who are unemployed and for whom disability has been established for the first time, for up to three consecutive months within a year from the date of disability establishment, must provide an individual rehabilitation and habilitation program for disabled individuals or a conclusion of the medical advisory commission.

The decision to conclude a contract for the provision of social services shall be made by the head of the territorial center within 5 business days after receiving the last document (information) required for the provision of social services.

The department is engaged in the adaptation of disabled individuals to work activities.

Volunteers assist in the organization of the department's activities.

Cultural and mass events, excursions, walks, and joint recreation are organized.

Transportation for recipients of social services enrolled in the department for social rehabilitation, habilitation of disabled individuals, and the elderly is provided daily on working days.

Personal Assistant Services are provided to expand the opportunities for people with disabilities and their active participation in society, including engagement in socially useful activities. The personal assistant services ensure an individualized approach in each specific case.

These services include providing assistance to disabled individuals in organizing and carrying out independent and self-reliant living, including support in acquiring self-care skills, help in planning and organizing daily life, making decisions in various life situations, establishing communication with other people, and other types of assistance. This may include searching for necessary information, filling out documents, visiting workshops, the department for social rehabilitation and habilitation of disabled individuals, concerts, exhibitions, providing first aid, calling a doctor, informing relatives about health status, etc.

This type of service is intended for the following categories of individuals:

For disabled individuals with severe limitations in the ability to move independently and/or orient themselves, corresponding to FC 4, and/or moderate or severe limitations in the ability to control their behavior, corresponding to FC 2 or FC 3, living together with able-bodied relatives who are legally obligated to support them.

Note: for this category of individuals, the service will be provided up to 20 hours per month.

For disabled individuals with limitations in life activities (mobility and/or orientation) corresponding to FC 4, living separately from able-bodied relatives legally obligated to support them, and for single disabled individuals.

Note: for this category of individuals, the service will be provided up to 40 hours per month.

For disabled individuals with limitations in life activities (behavioral control) corresponding to FC 2 or FC 3, living separately from able-bodied relatives legally obligated to support them, and for single disabled individuals.

Note: for this category of individuals, the service will be provided up to 60 hours per month.

To conclude a contract for the provision of personal assistant social services, a citizen must submit the following documents:

identity document of the citizen;
a document of the established sample on the right to benefits (disability certificate, Great Patriotic War veteran certificate, etc.) for citizens belonging to the category of beneficiaries;
consent to the processing of special personal data in cases provided for by personal data legislation;
written application (established sample);
individual rehabilitation, habilitation program for a disabled person, individual rehabilitation program for a disabled child, or conclusion of the Medical Advisory Commission – for Group I and II disabled persons, disabled children under 18 years of age who have completed the educational program for special education at the level of general secondary education for persons with intellectual disabilities.

Territorial center employees shall conduct an assessment of the citizen's living conditions within 3 working days from the date of application,

with the compilation of an act determining the individual need for social services.

The decision to conclude a contract for the provision of social services is made by the head of the territorial center within 5 working days after receiving the last document (information) required for the provision of social services.

Activities of the department (with elderly citizens):

identification and differentiated accounting of relevant citizens;
processing of necessary documents for the provision of social services;
organization of leisure activities and creation of conditions conducive to communication and maintenance of an active lifestyle for elderly citizens;
formation and provision of activities for interest groups and clubs;
preparation and conduct of cultural and mass events, sports and health improvement activities, holidays, contests, exhibitions;
involvement of volunteers in providing social services to elderly citizens and disabled persons by organizing interest groups, workshops, communication clubs, promoting the activities of volunteer groups and self-help and mutual support groups;
provision of other social services according to the list;
provision of day care services.

To conclude a social services agreement, a citizen must provide the following documents:

identity document of the citizen;
a document of the established form on the right to benefits (disability certificate, World War II veteran certificate, etc.) for citizens belonging to the category of beneficiaries;
consent to the processing of special personal data in cases provided for by personal data legislation;
a written application (of the established form);
a medical certificate of health status and (or) a conclusion of the Medical Advisory Commission containing information about the absence of medical contraindications for the provision of social services in a semi-inpatient social service setting.

Within 3 working days from the date of application, the employees of the territorial center will assess the living conditions of the citizen, draw up an act determining the individual need for social services, and request from state bodies and state organizations:

information about the residential premises occupied in the given locality, place of residence and family composition (indicating information about the place of residence and family composition) – regarding the person applying for social services;
information on the amount of pension received for the month preceding the month of application for social services – for single incapacitated citizens applying for social services;

information confirming that the citizen applying for social services is not a recipient of rent under a rent agreement or a life annuity agreement (with the exception of a life annuity agreement concluded with the local executive and administrative body) – for unemployed citizens aged 60 and over, who have reached the generally established retirement age, are eligible for a state pension, and are among single disabled citizens.

The decision to conclude an agreement for the provision of social services is made by the head of the territorial center within 5 working days after receiving the last document (information) required for the provision of social services.

The "Sanatorium at Home" service is a unique form of rehabilitation and non-drug assistance for elderly citizens, aimed at extending active longevity in their homes.

The provision of this service is aimed at providing citizens with social and health-improving services that contribute to improving their health, recovery after illnesses and injuries, as well as the prevention of chronic diseases and dementia.

The "Sanatorium at Home" service is provided on Fridays (16⁰⁰ –17⁰⁰)

Social services for elderly citizens in the department are provided free of charge, with the exception of the social services "Training in computer literacy, including mastering social networks, making payments on the Internet", "Ensuring the operation of interest clubs".

Free of charge for persons with disabilities of group I and II, low-income unemployed citizens aged 60 and over, who have reached the generally established retirement age, are eligible for a state pension, and are among single disabled citizens.

On the terms of partial payment for unemployed citizens aged 60 and over, who have reached the generally established retirement age, are eligible for a state pension, are among single disabled citizens, whose per capita income does not exceed 200 percent of the officially approved subsistence minimum budget per capita. The amount of partial payment for a single citizen is 60 percent of the social service rate (0.46 rubles/hour).
On the terms of full payment for unemployed citizens aged 60 and over, who have reached the generally established retirement age, are eligible for a state pension (0.77 rubles/hour).

The social services "Computer literacy training, including mastering social networks, making payments on the Internet", "Ensuring the operation of hobby groups", "Ensuring the operation of hobby clubs" can be provided in the form of remote social services.
